Eligibility - Population(s) Served: People who have had an acute neurological event (less than 6 months post injury): ischemic or hemorrhagic stroke, neuro trauma, anoxic brain injury, infectious neurological injury, spinal cord injury * must have alpha-FIM and/or FIM (Functional Independence Measure) completed * must require 2 or more services (physiotherapy, occupational therapy, speech language therapy) * must be able to participate in an active rehabilitation program * must identify goals that can be achieved within the framework of outpatient rehabilitation * must have established transportation * must have social support for completion of home programming (if cannot be performed independently) * must adhere to cancellation policy (excluding inclement weather) * must have a home address within Ontario Health atHome Central area * will consider prior rehabilitation services received (i.e. inpatient, outpatient, private)

Exclusion Criteria
Medical: progressive neurological disease as primary diagnosis * chronic event (greater than 6 months post injury)
Other: single service required * insufficient tolerance for participation in active outpatient rehab program * goals not achievable within the framework of outpatient rehab * no transportation * no social support for completion of home programming (if cannot be performed independently) * home address outside of Cental LHIN * concurrent direct intervention provided by another OHIP-funded service (convalescence, HC&C rehab services) * concurrent direct intervention provided by private therapist
