Description of Services: A specialized program providing care and active management to medically complex patients requiring an in-patient hospital stay and cannot be safely managed in a community setting.  This program offers on-site physician/nurse practitioner to manage patient's medical needs. This program also provides a holistic care approach delivered by our interdiscipilnary care team.

Participation: Able to participate in physical therapy

Medical: Chemotherapy and radiation, Dialysis (hemo and peritoneal), chest tubes, ventilation, BiPAPTM, NG Tubes
Infectious: Tuberculosis, Clostridioides difficile
Bariatric: Weight over 300 lbs or 136 kgs

Respiratory Care Oxygen: Bi-Pap * C-Pap * Continuous Oxygen * Intermittent Oxygen * Tracheostomy
IV Therapy: Peripheral * PICC
Nutrition: GJ Tubes
Skin Condition: Specialized Wound Care
Continence: Indwelling Urinary Catheters * Intermittent Urinary Catheters
