Description of Services: | A specialized program providing care and active management to medically complex patients requiring an in-patient hospital stay and cannot be safely managed in a community setting. This program offers on-site physician/nurse practitioner to manage patient's medical needs. This program also provides a holistic care approach delivered by our interdiscipilnary care team. | ||
