Description of Services: Short-term complex medical management program for adults who need ventilators to breathe * care team trains patients and caregivers on use of ventilation in a home setting and how to handle emergency situations * follow-up reassessments provided regularly after discharge, followed by every 12 months * short-stay re-admission offered if necessary
Requirements
Fees: Service and standard ward accommodation covered by OHIP * inpatients unable to return to independent living after program and are waiting for placement in a facility providing an appropriate level of care must pay a daily chronic care co-payment
Eligibility - Population(s) Served: Adults 18 years and older who require mechanical ventilation for all or part of the day * must be medically stable * must be cognitively stable (or have a documented substitute decision-maker) * must be willing to participate in the program * must be able to direct own care * must be able to return to the community with established discharge support
