| Medical: | patients requiring more than minimal assistance for transfers/ambulance * cognitive difficulties preventing patient participation | ||
| Other: | patients able to receive services in a long-term care home or retirement residents * patients requiring only one service * previous admission to Geriatric Day Hospital in last 2 years with no significant change in status | ||
