Description of Services: Hospital-based outpatient medical rehabilitation program for older adults with complex health problems * focus is on reducing the need for emergency admission to hospital as well as decreasing the need for admission to long-term care * aims to increase independence in the community * once referred, clients must be medically cleared by service geriatrician or care of the elderly physician * comprehensive interdisciplinary approach provided by a physician, nurse, dietitian, physiotherapist, occupational therapist, pharmacist, recreation therapist, social worker and rehabilitation therapy assistants * program length of stay for clients is determined after the initial assessments are completed by the Specialized Geriatric team * team works with community partners to ensure clients linked to appropriate community programs to maintain gains

Eligibility - Population(s) Served: Adults 65 years and older who have multiple medical, functional, or psychosocial problems * must be able to participate safely within a group setting * must be able to learn from education sessions * must not be a resident of a long term care facility * family/support person strongly encouraged
