Access to information is the first step to ensuring access to services.


Rehab Finder provides a searchable database of rehabilitation programs/clinics/services provided by hospitals and Home and Community Care across the Greater Toronto Area.


Rehab programs/clinics/services can be searched by:

•   Age group
•   Rehabilitative Condition/Population
•   Level of Rehabilitative Care
•   External Referrals Accepted
•   Cost to Client
•   Special Needs
 

Information provided includes:

•   Program Details (organization, description of services)
•   Requirements (fees, eligibility, application, accessibility, languages, area(s) served)
•   Contact Information
•   Rehabilitative Care Details (level of care, exclusion criteria, special needs, etc.)

 

The database lists:

•   Community-Based Rehabilitative Care
•   Condition or Population Based Rehabilitative Care
•   Hospital-Based Outpatient Rehabilitative Care
•   Inpatient Rehabilitative Care

Information is provided only for the publicly-funded organizations that are members of the GTA Rehab Network.


Rehab Finder data is integrated from thehealthline.ca database. Records are reviewed at least once annually and must meet inclusion criteria and data standards.


*Updating and Adding Services to Rehab Finder: Service providers who are members of the GTA Rehab Network can submit updates or new rehab records to Rehab Finder anytime by logging in to the Service Provider Access Tool. Your records will be reviewed by data administrators before the changes are live.
